Description
Spanning the anterolateral thoracoabdominal wall, the external oblique (musculus obliquus externus abdominis) appears as a broad superficial sheet with inferomedial fiber direction, running from the external surfaces of ribs 5 to 12 toward its anterior aponeurosis. Along the lateral thorax, serratus anterior is highlighted on the outer rib cage, its digitations interlocking with the superior slips of external oblique at the midaxillary line. Posteriorly, the scapula and proximal humerus frame the shoulder region, with serratus anterior tracking from the lateral ribs to the medial border of the scapula. That serratus anterior and external oblique share adjacent rib attachments matters when you teach surface anatomy and interpret lateral chest pain or trauma: rib fractures, intercostal nerve irritation, and muscle strain can mimic each other. Serratus anterior dysfunction from long thoracic nerve injury produces medial scapular winging and loss of smooth upward rotation during abduction, while external oblique injury or denervation affects trunk rotation and can weaken the anterior abdominal wall, increasing susceptibility to incisional or Spigelian hernias.
