Description
Arising from the popliteal artery near the adductor hiatus, the superior medial genicular artery courses anteriorly and medially across the distal femur, hugging the periosteum just superior to the medial femoral epicondyle. From a medial perspective, it runs deep to the medial collateral ligament (tibial collateral ligament) and in close proximity to the tendon of adductor magnus posteriorly and the distal vastus medialis anteriorly. Small muscular and periarticular branches feed into the genicular anastomosis around the knee, with accompanying venae comitantes and adjacent articular nerve twigs tracking along the vessel. Relationships are tight. This angle matters when you need to understand how medial-sided stabilizers and vascular supply coexist at the knee. During medial collateral ligament repair or reconstruction, medial meniscal surgery, or medial parapatellar arthrotomy, the superior medial genicular artery is a recognized source of periarticular bleeding because it crosses the operative field near the medial epicondyle and capsuloligamentous complex. Its contribution to the genicular network also becomes clinically relevant in peripheral arterial disease and after femoropopliteal bypass, where collateral flow around the knee can preserve distal perfusion.
