Description

Arcing across the anteromedial aspect of the knee, the superior medial genicular artery is traced as it courses distal to the adductor tubercle and sweeps anteriorly over the medial femoral condyle toward the patellar anastomosis. The femur, tibia, fibular head, and patella frame the joint, with the vessel running superficial to the periosteum and deep to the investing fascia near the medial retinacular tissues. Medial and lateral muscle bellies bracket the joint line, while tendinous and capsuloligamentous structures appear as pale bands spanning from distal femur to proximal tibia. Small caliber arterioles and accompanying veins may be shown converging around the condyle. A tight corridor. This anteriorly oriented depiction matters because the genicular network often becomes the operative field, not just background anatomy, during medial parapatellar approaches, total knee arthroplasty exposure, and medial retinacular releases. The superior medial genicular artery is a common source of periarticular bleeding when the medial capsule and synovium are elevated off the femoral condyle, and it can also contribute to recurrent hemarthrosis after arthroscopy if injured. Its relationship to the medial femoral condyle and patella makes it a practical landmark when teaching the patellar anastomosis and collateral circulation around the knee in the setting of popliteal artery disease.
