Description
Arising from the popliteal artery proximal to the knee joint line, the superior medial genicular artery courses anteriorly and medially along the distal femur, passing deep to the tendon of adductor magnus near the adductor tubercle. Its branches sweep across the medial aspect of the distal femur and femoral condyle, then contribute to the peripatellar and genicular arterial anastomosis on the anterior knee. Surrounding relationships in an anterior projection logically include the distal femur, proximal tibia, patella, quadriceps tendon, patellar ligament, and the medial collateral ligament region, with hamstring and adductor tendons positioned more posterior and medial. For medial knee surgery and trauma, this vessel matters because it marks where the popliteal system transitions into a dense collateral network that can maintain distal perfusion when the superficial femoral or popliteal artery is compromised, and it can also be a source of periarticular bleeding. During a medial parapatellar approach for total knee arthroplasty, exposure around the medial femoral condyle and vastus medialis can place small genicular branches at risk, and iatrogenic injury may contribute to postoperative hemarthrosis. Clear identification helps when teaching why genicular collaterals enlarge in chronic occlusive disease. Small artery, big consequence.
