Description
Plantar anatomy is presented with the metatarsals and phalanges seen from the sole, the calcaneus positioned posteriorly and the tarsal region proximal to the metatarsal bases. The deep plantar arch courses transversely across the proximal metatarsals, from which the plantar metatarsal arteries arise and run anteriorly (distally) in the intermetatarsal spaces toward the digits. Adjacent plantar venous channels accompany the arteries, and fibrous connective tissue bands consistent with the plantar fascia span superficially across the midfoot. Orientation is plantar, not dorsal. Clinically, the deep plantar arch and its plantar metatarsal branches matter any time forefoot perfusion is at risk, from diabetic foot ulceration and critical limb ischemia to traumatic lacerations across the midsole. Surgeons rely on this spatial map during plantar approaches for interdigital (Morton) neuroma decompression, metatarsal osteotomy, and soft tissue reconstruction, where injury to the plantar metatarsal arteries can compromise toe viability or create a difficult-to-control bleed. A clear plantar perspective also helps correlate anatomic supply territories with Doppler examination and angiographic findings in peripheral arterial disease. Use this plate for teaching lower limb vascular anatomy in gross anatomy and podiatric medicine curricula, and for labeling in textbooks or surgical atlases covering plantar incisions, flap design, and forefoot complications in diabetes.
