Description
Arising from the posterior tibial artery behind the medial malleolus, the medial and lateral plantar arteries course anteriorly across the plantar aspect of the foot and feed the plantar metatarsal arteries within the deep plantar arch. From these channels, the plantar digital arteries run distally along the medial and lateral borders of each toe, closely applied to the phalanges and separated from the plantar skin by the fibrous digital sheaths and plantar aponeurosis. Distally, each proper plantar digital artery reaches the pulp at the tip of the hallux and lesser toes, where terminal arterioles form dense capillary beds. Bones of the distal tibia and fibula, talus, calcaneus, tarsals, metatarsals, and phalanges provide the bony scaffold that makes the vessel paths easy to track in medial profile. Small but specific. Medial exposure of these vessels matters when you need to think like a surgeon or a wound specialist. The plantar digital arteries are the dominant inflow to the toe pads, so they directly condition healing after hallux valgus correction, toe amputations, and plantar flap design, and they are a frequent culprit in toe ischemia in diabetes and peripheral arterial disease when distal runoff is poor. This perspective also supports bedside correlation with Doppler assessment along the plantar-medial forefoot and helps explain why plantar lacerations near the metatarsophalangeal joints can bleed briskly even when dorsal flow seems preserved.
