Description
Plantar digital arteries are traced across the left sole from the posterior tibial artery as it passes posterior to the medial malleolus and divides into the medial and lateral plantar arteries. Distally, the lateral plantar artery sweeps laterally and then curves medially to form the plantar arterial arch, which gives rise to plantar metatarsal arteries and then proper plantar digital arteries running along the medial and lateral margins of each toe. Proximal caliber is greatest near the tarsal tunnel region, then the vessels taper into arteriolar branches toward the distal phalanges. Small branches cross between adjacent rays. This plantar distribution matters when you are thinking about perfusion of the toes and the characteristic patterns of ischemia in peripheral arterial disease and diabetes, where digital ulcers often appear at the distal tufts and along pressure points. For surgeons, the relationship of the posterior tibial artery to the flexor retinaculum and the early branching into plantar vessels underlies safe dissection during tarsal tunnel release and when planning incisions for plantar fasciotomy. Angiosome-based revascularization strategies also depend on knowing which plantar territories are supplied by the medial versus lateral plantar system. Tiny vessels, big consequences.
