Description
Oriented to the plantar surface of the right foot, the plantar digital arteries course distally from the plantar arterial arch and common plantar digital branches to become proper plantar digital arteries along the medial and lateral margins of each toe. Metatarsals and phalanges form the deep scaffold, while the vessels track in the soft tissue plane superficial to the flexor tendons and plantar plates, hugging the fibrous digital sheaths as they approach the toe pads. Proximally, the calcaneus, talus, and tarsal bones anchor the hindfoot, with arterial and venous channels threading around the ankle into the sole. Red arterial pathways contrast with accompanying blue veins, emphasizing paired neurovascular bundles at the sides of the digits. Plantar digital arterial anatomy matters when tissue perfusion is the question. Toe ischemia in peripheral arterial disease, diabetic microangiopathy, and critical limb-threatening ischemia often declares itself at the distal digits, and knowing where the proper digital arteries run helps correlate a plantar ulcer or gangrenous tip with the level of inflow compromise.
