Description
Seen from posterior, the descending thoracic aorta crosses the aortic hiatus of the diaphragm to become the abdominal aorta, running inferiorly in the midline just anterior to the vertebral bodies. The inferior vena cava parallels it on the patient’s right, while the paired kidneys sit posterolateral to the great vessels, their hila oriented anteromedially toward the renal arteries and veins. Segmental branches course laterally toward the retroperitoneum, and a yellow autonomic nerve network tracks alongside the vessels, consistent with the aortic plexus and sympathetic trunks. Pelvic bones and lower ribs anchor the topography. Posterior orientation clarifies what is actually “behind” the aorta: vertebrae, diaphragmatic crura, and the retrocrural space where lymphatics and autonomic nerves cluster around the vessel. That relationship matters in open and endovascular repair of abdominal aortic aneurysm, where proximal control near the renal arteries and dissection around the left renal vein, lumbar arteries, and sympathetic plexus drives both bleeding risk and postoperative issues such as retrograde ejaculation from autonomic injury. It also supports interpretation of posterior approaches and cross sectional imaging, where the aorta’s position relative to the inferior vena cava and renal pedicles helps localize retroperitoneal hematoma or lymphadenopathy.
