Posteriorly, the arterial tree is traced from the common and external iliac arteries into the femoral artery as it courses through the thigh toward the adductor canal, where it lies deep to sartorius and adjacent to adductor longus and adductor magnus before becoming the popliteal artery at the adductor hiatus. The profunda femoris (deep femoral) artery branches posterolaterally from the proximal femoral artery, sending perforating arteries through adductor magnus to supply the posterior compartment. Paired deep femoral veins and the femoral vein run closely alongside their arterial counterparts, while the popliteal vessels align centrally in the popliteal fossa distal to the hiatus. Deep. Because the femoral artery is usually taught from an anterior approach, a posterior perspective clarifies the segment that can be deceptively hidden as it transits the adductor canal, a common site of entrapment-like symptoms and a key landmark when distinguishing true superficial femoral artery disease from popliteal-level pathology. For vascular and trauma surgery, the relationship of the femoral and profunda systems to adductor magnus explains why penetrating posterior thigh injuries can cause brisk hemorrhage yet spare the more anterior femoral pulse, and it frames collateral routes that maintain distal perfusion when the superficial femoral artery occludes. The profunda matters here.
