Description
Arising from the femoral artery in the proximal anterior thigh, the deep femoral artery (profunda femoris) is shown coursing posteriorly and laterally behind the femoral vessels as it enters the deep compartment. From its proximal segment, the medial and lateral circumflex femoral arteries typically diverge to wind around the femoral neck, while perforating arteries pass posteriorly through adductor magnus to reach the posterior thigh musculature. Superficial venous return is suggested by the great saphenous vein running along the medial leg and thigh, with deeper paired veins accompanying the femoral arterial trunk proximally within the femoral triangle. For teaching and operative planning, the profunda femoris matters because it is the dominant arterial source to the posterior thigh and a major collateral pathway when the superficial femoral artery is stenosed or occluded in peripheral arterial disease. Misidentifying it during common femoral endarterectomy or femoral access can compromise thigh perfusion, and its proximal branches sit near the femoral neck where hip fracture patterns and orthopedic approaches intersect with the lateral circumflex femoral system. Expect anatomic variation. The origin level and branching pattern often shift relative to the inguinal ligament and the adductor longus, which is why this morphology-based depiction is useful. Use this illustration in lower-limb anatomy and vascular surgery modules, in angiography or duplex ultrasound teaching to orient learners to profunda versus superficial femoral flow targets, and in publications discussing femoral access complications, hematoma formation, or collateralization in chronic limb-threatening ischemia.
