Description
Centered in the anterior cranial fossa, the ethmoid bone is seen from above with the cribriform plate forming a perforated roof over the nasal cavity and the crista galli projecting superiorly in the midline. Multiple olfactory foramina pepper the plate on either side of the crista galli, while the ethmoidal labyrinths lie lateral to the midline, articulating with the frontal bone and the lesser wings of the sphenoid. Posterior to the ethmoid region, the body of the sphenoid contributes to the middle cranial fossa, and farther posterior the occipital bone frames the foramen magnum. Orientation is internal, endocranial. This superior endocranial view matters because it aligns the olfactory pathways with the skull base anatomy you confront in both trauma and endoscopic skull base surgery. Fracture through the cribriform plate can shear fila olfactoria and tear the dura, a classic setup for anosmia and cerebrospinal fluid rhinorrhea with risk of ascending meningitis. The crista galli is a clean landmark for midline, and its attachment to the falx cerebri explains why midline shifts and dural tension can be discussed from this angle. Use it for teaching the anterior cranial fossa in gross anatomy, neuroanatomy, and ENT modules, or as a figure in publications covering cribriform plate fractures, CSF leak workup, or endoscopic transcribriform approaches to anterior skull base lesions.
