Description
Positioned in a lateral skull projection, the ethmoid bone is seen interposed between the frontal bone superiorly and the sphenoid posteriorly, with its orbital plate (lamina papyracea) forming the medial wall of the orbit just medial to the globe. Inferior and anterior to the cribriform plate region, the ethmoidal labyrinth contributes to the lateral wall of the nasal cavity, where the superior and middle nasal conchae (turbinates) project medially toward the nasal septum. Anteriorly, the ethmoid articulates with the nasal bone and maxilla, while ethmoidal air cells lie between the orbit laterally and the nasal cavity medially. Thin bone. Tight spaces. For clinical correlation, the lateral relationship between the lamina papyracea and ethmoid air cells matters when tracking spread of acute ethmoiditis into the orbit, a common pathway for medial orbital cellulitis and subperiosteal abscess in children. Endoscopic sinus surgery also works along this paper-thin boundary, where a small breach risks orbital fat herniation or extraocular muscle injury, and the cribriform plate region remains the high-risk zone for cerebrospinal fluid leak and postoperative meningitis. This view also helps anchor the ethmoid’s position when teaching why fractures of the medial orbital wall can present as orbital emphysema after nose blowing.
