Description
Centered at the cranial base, the sphenoid is presented as the butterfly-shaped keystone of the skull, with the body in the midline and the greater wings projecting laterally toward the temporal fossae. From this superior intracranial perspective, the lesser wings form the posterior boundary of the anterior cranial fossa and converge medially on the anterior clinoid processes, flanking the chiasmatic sulcus and the region of the sella turcica. Posteriorly, the dorsum sellae and posterior clinoid processes rise at the junction with the clivus, while the optic canals and the superior orbital fissures sit anterolaterally as paired gateways toward the orbit. Bony landmarks dominate. This view matters because the sphenoid’s foramina and ridges map directly onto high-stakes neurovascular corridors: the optic canal for the optic nerve and ophthalmic artery, the superior orbital fissure for cranial nerves III, IV, V1, and VI, and the foramina of the greater wing that transmit branches of the trigeminal nerve and the middle meningeal artery. Surgical planning around the sella, whether for transsphenoidal pituitary approaches or anterior clinoidectomy in paraclinoid aneurysm work, depends on recognizing the relationships between the anterior clinoid, optic strut, and the parasellar carotid groove. Small shifts in bony anatomy can change exposure. It is a frequent source of complication.
