Anterior soft tissues of the male hand are rendered with the skin removed to expose the proximal commissural ligament, more formally the deep transverse metacarpal ligament, spanning the palmar aspect of the metacarpophalangeal region at the level of the web spaces. The band runs transversely between adjacent metacarpal heads, lying deep to the superficial palmar fascia and superficial to the interossei, while remaining closely related to the volar plates and the flexor tendon sheaths. Distally, the digital rays and their tendon pathways are suggested toward the proximal phalanges, while proximally the structure sits just distal to the carpal level and the central palm. A small structure. Big consequences. Understanding the deep transverse metacarpal ligament matters any time you are thinking about web-space stability and controlled finger abduction. In open reduction of metacarpophalangeal joint dislocations, the ligament’s relationship to the volar plate and flexor apparatus explains why a seemingly simple dorsal dislocation can become irreducible when the volar plate and adjacent soft tissues interpose around the metacarpal head, and why careful release in the correct plane prevents collateral damage. Tightening or scarring across this transverse band also contributes to post-traumatic commissural contracture and complicates syndactyly release, because the ligament anchors the bases of the digits and resists widening of the interdigital space.
