Description
Oriented in an inferior (basal) view, the encephalon is presented from the ventral surface of the cerebrum to the posterior cerebellum, with the brainstem centered in the midline. The optic chiasm and optic tracts occupy the anterior basal diencephalon, while the oculomotor nerves emerge from the interpeduncular fossa of the midbrain just posterior to the mammillary region. Along the pontomedullary junction, the trigeminal nerve exits anterolaterally from the pons, and the facial and vestibulocochlear nerves appear more caudolateral at the cerebellopontine angle; the glossopharyngeal, vagus, and accessory rootlets arise as a series along the postolateral sulcus of the medulla, with the hypoglossal nerve emerging more medial, between pyramid and olive. Clear midline anatomy. This basal perspective matters because it aligns the cranial nerve root exit zones with the skull base foramina encountered in clinic and in the operating room. Correlating the VII and VIII nerve complex at the cerebellopontine angle with the internal acoustic meatus is central when teaching vestibular schwannoma anatomy and the pattern of progressive unilateral sensorineural hearing loss and facial weakness. The lower cranial nerve rootlets along the medulla also map directly to jugular foramen syndromes and to surgical corridors used in far-lateral approaches around the foramen magnum.
