Description
Dominating the central cranial base, the sphenoid (os sphenoidale) is highlighted against the surrounding skull in a true inferior perspective, with the body lying in the midline and the greater wings projecting laterally toward the temporal and zygomatic regions. Posterior to the sphenoid, the occipital bone frames the foramen magnum, while the sphenoid’s basilar relationship to the clivus is implied at the posterior margin of its body. Extending inferiorly from the junction of body and greater wings, the pterygoid processes descend as paired plates, positioned medial to the infratemporal surfaces and lateral to the nasopharyngeal midline. Small apertures along the sphenoid and adjacent bones punctuate the skull base. This is a dense piece of anatomy. An inferior skull base view matters because it’s where the sphenoid becomes a hub for neurovascular transit and surgical corridors: the foramen ovale and foramen spinosum open in the greater wing for V3 and the middle meningeal artery, and the pterygoid region anchors approaches that pass through the infratemporal fossa. Surgeons orient to these landmarks during endoscopic endonasal access to the central skull base and when controlling epistaxis via the internal maxillary artery branches in the pterygopalatine fossa. Fractures here can be subtle yet dangerous, and a line through the greater wing can intersect the middle meningeal groove and raise concern for epidural hematoma. Use this plate for head and neck anatomy labs, neurosurgical or ENT atlases discussing skull base approaches, and radiology teaching that correlates CT bone windows with foramina of the sphenoid and adjacent cranial base. It also fits well in exam prep material on the pterygoid process, foramina, and the sella turcica as a midline orientation point, even when viewed indirectly from below.
