Description
Centrally within the anterior cranial base, the os sphenoidale is presented in anterior view, with the body sitting posterior to the nasal cavity and between the orbits. From the body, the greater wings extend superolaterally toward the orbital apices, while the lesser wings form a sharp superior margin that contributes to the posterior roof of the orbit. Inferiorly, the paired pterygoid processes descend from the junction of body and greater wing, dividing into medial and lateral pterygoid plates with the pterygoid hamulus curving posterolaterally at the distal end of the medial plate. Superior to the anterior surface, the sella turcica region is referenced anatomically by the tuberculum sellae and dorsum sellae, even though it sits on the superior aspect of the body. An anterior sphenoid view anchors several high-yield corridors: the superior orbital fissure between the lesser and greater wings and the optic canal within the lesser wing sit at the orbital apex, where trauma, orbital decompression, or tumor extension can threaten cranial nerves II, III, IV, V1, V2, and VI. Pterygoid process morphology matters for transnasal and transmaxillary approaches to the pterygopalatine fossa, and for understanding the attachment and line of pull of the medial and lateral pterygoid muscles in temporomandibular joint dysfunction. Small errors here mislead learners fast. It is a hard bone to teach.
