Description
Positioned on the posterolateral aspect of the skull, the os temporale is identified by the mastoid process projecting inferiorly and posteriorly, just behind the external acoustic meatus and lateral to the occipitomastoid suture. Superior to the mastoid region, the squamous part of the temporal bone meets the parietal bone at the squamous suture, while the posterior margin approaches the occipital bone at the occipitomastoid suture. Medially, the petrous part is implied as the dense wedge of bone that houses the inner ear and forms a major component of the cranial base. Blue color differentiation isolates the temporal bone from adjacent cranial bones for rapid orientation. Posterior temporal bone anatomy matters because it anchors both neuro-otologic landmarks and surgical corridors. The mastoid process contains mastoid air cells that communicate with the middle ear via the aditus ad antrum, a relationship central to mastoiditis and to the rationale for cortical mastoidectomy. Just anterior to the mastoid, the stylomastoid foramen transmits the facial nerve (CN VII), and the mastoid segment of the facial canal lies at risk during posterior approaches to the middle ear. Small errors here have consequences. This view also helps distinguish mastoid enlargement patterns, which correlate with pneumatization and chronic otitis media history.
