Description
Centered in the lumbar spine, the third lumbar vertebra (L3) is presented in lateral profile within the full vertebral column, from the cervical region superiorly through the thoracic and lumbar segments to the sacrum and coccyx inferiorly. The L3 vertebral body sits anterior to the vertebral arch, with the pedicles and laminae enclosing the vertebral foramen posteriorly, and the spinous process projecting posterior and slightly inferior. Superior and inferior articular processes frame the zygapophysial (facet) joints, while transverse processes extend laterally from the vertebral arch. Intervertebral discs separate adjacent vertebral bodies and maintain the lumbar lordosis. A lateral view is the clearest way to teach sagittal alignment and load transfer through the lumbar column, and it places L3 in context relative to the thoracic kyphosis above and the lumbosacral junction below. L3 is a common reference level for degenerative disc disease and central canal stenosis, and its posterior elements help explain how facet arthropathy can narrow the lateral recess and contribute to L3 or L4 radicular symptoms. Bone landmarks at this level also matter during lumbar puncture planning, where surface anatomy guides needle placement toward the L3 to L4 or L4 to L5 interspace to avoid the conus medullaris.
