Nearby, the flexor hallucis longus tendon passes to the hallux on the medial plantar side, while quadratus plantae sits proximal and lateral, inserting onto flexor digitorum longus to redirect its line of pull. The metatarsals and proximal phalanges frame the muscular slip paths. Functionally, the lumbricals coordinate metatarsophalangeal flexion with interphalangeal extension by tensioning the extensor hood, so their loss shifts the balance toward long extensor dominance and contributes to clawing of the lesser toes. Weakness and atrophy of these intrinsic muscles are common in diabetic peripheral neuropathy and chronic denervation, and the resulting MTP hyperextension with PIP flexion can drive plantar plate overload, transfer metatarsalgia, and dorsal corn formation.
