Description
Viewed from the posterior aspect, the adult male bony pelvis is presented as a ring formed by the right and left os coxae articulating with the sacrum at the sacroiliac joints. The iliac crests and posterior superior iliac spines sit superiorly, with the iliac fossae curving anteriorly away from view, while the ischial tuberosities occupy the most inferior and posterior prominence on either side of the pelvic outlet. Lateral to the sacrum, the greater sciatic notches sweep inferiorly toward the ischial spines, and the acetabula face laterally, their rims framing the hip joint sockets above the obturator foramina. Symmetry matters. This posterior projection is the clearest way to teach surface and deep landmarks used in pelvic trauma patterns and posterior approaches to the hip. The relationship between the posterior ilium, the greater sciatic notch, and the ischial spine anchors the course of the sciatic nerve as it exits the pelvis through the greater sciatic foramen, a key concern in posterior hip dislocation and during deep gluteal injections. Clear visualization of the sacroiliac region also supports discussion of sacroiliitis and the mechanics of pelvic ring instability, where disruption may occur at the pubic symphysis anteriorly with a corresponding posterior lesion at the sacroiliac joint or iliac wing. Use this artwork in gross anatomy teaching on the pelvic girdle, orthopaedic and radiology modules on pelvic fractures and sacroiliac joint pathology, and surgical education illustrating posterior hip approaches and the safe zones around the greater sciatic notch.
