Description
Right lateral thoracic anatomy centers on the heart with the superior mediastinal venous confluence in situ. The right and left brachiocephalic veins are positioned superior to the atria, coursing obliquely and medially to form the superior vena cava, which descends on the right side of the ascending aorta before entering the right atrium. Anterior to these great veins, the sternum and costal cartilages frame the mediastinum, while the aortic arch and its proximal branches sit posterior and leftward relative to the caval axis. Color coding separates arterial from venous pathways. A right-sided view clarifies relationships that drive access and complication patterns: the brachiocephalic veins lie immediately posterior to the manubrium, so mediastinal masses, substernal goiters, and indwelling central venous catheters can narrow the lumen and produce upper extremity and facial venous congestion from superior vena cava syndrome. Angles matter. The left brachiocephalic vein takes a longer, more horizontal course across the superior mediastinum, a frequent site of catheter impingement and thrombosis, while the right brachiocephalic vein runs shorter and more vertical into the SVC, informing line trajectory from the right internal jugular or subclavian approach. For teaching, this projection also helps distinguish venous return anatomy from the adjacent aortic arch and pulmonary trunk.
