Description
Arising proximally from the supraglenoid tubercle and the coracoid process, the long and short heads of the biceps brachii converge into a single fusiform belly on the anterior aspect of the arm. Distally, the muscle narrows to the biceps tendon, which courses inferiorly across the cubital fossa to insert on the radial tuberosity, with the bicipital aponeurosis fanning medially into the antebrachial fascia. Surrounding anterior compartment muscles, including brachialis deep to the biceps and coracobrachialis proximomedially, are shown in neutral tones to clarify borders and planes. Tendinous structures in the forearm and hand extend distally, providing continuity from arm to wrist. An anterior perspective is the clearest way to teach how biceps brachii functions as both a powerful supinator and an elbow flexor, because the line of pull to the radius and the medial expansion of the aponeurosis are easy to relate to palpable surface anatomy. Clinically, this anatomy frames the assessment of distal biceps tendon rupture, where loss of the normal anterior contour and weakness in supination follow retraction of the tendon, and it also supports safe venipuncture or surgical exposure in the cubital fossa by reinforcing where the bicipital aponeurosis lies superficial to the brachial artery and median nerve. A simple landmark. The two-headed proximal anatomy also explains common pain generators near the shoulder when the long head tendon is involved.
