Description
Anteriorly, the biceps brachii is presented as the dominant anterior compartment muscle of the upper arm, with its long head positioned more laterally and the short head more medially as they converge into a single fusiform belly over the anterior humerus. Proximally, the long head tendon is understood to course superiorly toward the intertubercular sulcus, while the short head tracks toward the coracoid process; distally, the tendon narrows toward the radial tuberosity with a medial expansion consistent with the bicipital aponeurosis toward the forearm fascia. Deep and lateral to the biceps, the brachialis and brachioradialis relationships are implied by the line of pull to the radius, and the humerus, radius, ulna, and hand bones (carpals, metacarpals, phalanges) anchor orientation at the elbow and wrist. Clinically, the anterior perspective is where you teach and recognize long head biceps tendinopathy and proximal tendon rupture, with the long head’s intra-articular origin and bicipital groove course explaining why shoulder pain can present with focal tenderness over the anterior proximal humerus. Distal biceps rupture is also best framed here because loss of the normal contour, weakness in forearm supination, and the tendon’s insertion on the radial tuberosity relate directly to the anatomy shown, and the bicipital aponeurosis can mask retraction and complicate palpation in partial tears.
