Description
Running along the anterior compartment of the left arm, the biceps brachii is exposed beneath the reflected skin and superficial fascia, its long head positioned more laterally and the short head more medially as they descend toward the cubital fossa. Proximally, the long head tendon tracks into the intertubercular sulcus of the humerus toward the supraglenoid tubercle, while the short head tendon courses from the coracoid process, the two bellies converging into a single distal tendon. Distally, that tendon passes anterior to the elbow to insert on the radial tuberosity, with a medial expansion forming the bicipital aponeurosis over the flexor–pronator mass. Portions of brachialis lie deep to the biceps on the anterior humerus, and the proximal forearm musculature is suggested beyond the elbow. Separating the long and short heads in an anterior view matters because many clinical problems localize to one component of the unit. Long head biceps tendinopathy and instability often relate to the bicipital groove and the transverse humeral ligament region, while rupture of the proximal long head can produce the classic distal muscle belly retraction seen in a Popeye deformity. At the elbow, the distal biceps tendon and its footprint on the radial tuberosity are frequent sites of partial tear or complete rupture after eccentric loading, and this view helps clarify why forearm supination strength drops more than elbow flexion.
