Description
Arising on the dorsum of the foot, the dorsal metatarsal arteries course distally between the metatarsal shafts and give off dorsal digital arteries that track along the dorsal aspects of the toes toward the distal phalanges. The vessels run superficial to the metatarsals and phalanges, closely related to the extensor hallucis longus and extensor digitorum longus tendons as they pass over the metatarsophalangeal joints. Medially, branches to the hallux align with the first metatarsal and proximal phalanx, while more lateral dorsal digital arteries mirror the second through fifth rays in parallel, tapering to fine arterioles near the nail beds. Small dorsal venous channels accompany the arterial tree. For teaching forefoot vascular territories, a dorsal view clarifies where dorsal flow complements plantar supply and where anatomic variation is common, including the caliber and presence of the first dorsal metatarsal artery. This matters in microvascular planning and in trauma, because dorsal lacerations, crush injuries, or fixation around the metatarsals can compromise distal perfusion to a toe even when plantar pulses remain present. It also frames the relationship between dorsal digital arteries and the extensor tendon compartments, a practical orientation when planning dorsal incisions and avoiding iatrogenic injury.
