Description
Running along the lateral aspect of the distal femur, the superior lateral genicular artery branches from the popliteal artery and courses anteriorly toward the lateral femoral epicondyle, hugging the periosteum just superior to the lateral condyle. From a lateral perspective you can follow it as it passes deep to the tendon of biceps femoris near the fibular head, then continues toward the peripatellar arterial anastomosis at the superolateral margin of the patella. Portions of the lateral tibial plateau, the head of fibula, and the lateral joint line sit inferior to this vessel, with articular cartilage capping the femoral condyle and tibial plateau at the weightbearing surfaces. Small accompanying veins parallel the arterial course. For operative anatomy, the superior lateral genicular artery is a consistent landmark during lateral approaches to the knee and distal femur, where it may be encountered during dissection around the lateral epicondyle or when working near the iliotibial tract insertion and biceps femoris tendon. Bleeding from a transected genicular branch can obscure the field during open reduction and internal fixation of distal femoral fractures or during lateral retinacular work around the patella, and preserving the genicular network supports periarticular perfusion after trauma. This territory matters. It is also relevant in planning periarticular flaps and in understanding why popliteal artery injury can compromise collateral circulation around the knee despite apparently intact distal pulses. Educators can place this plate into lower limb anatomy courses to teach the genicular anastomosis, or into orthopaedic and sports medicine teaching files when discussing lateral meniscal surgery corridors and risks near the fibular head.
